Support at Home Services

Community Therapy is an accredited Support at Home provider of Allied health services. Our Support at Home services are designed to help you remain safe, confident, and connected in your own environment.

We visit older adults and their families to identify strengths, set goals, and build practical skills. From home safety assessments and mobility strategies to daily living assistance and falls prevention, we provide tailored support that respects your preferences and maximises your independence. We want you to live at home your way!

Occupational Therapy

Occupational therapy is all about enabling participation — in everyday activities that matter most to you. Our occupational therapists assist people of all ages to build skills, adapt tasks, and modify environments so they can be safe, productive, and engaged.

Services include all types of assessments, equipment prescription, skill development, home modifications and falls prevention. We primarily deliver our OT services under the NDIS scheme (for adults and children) as well as the Support at Home Scheme (for older adults).

Whether you’re managing a neurological condition, recovering from injury, or seeking greater confidence with daily tasks, our team is here to listen to what you find important.

NDIS Allied Health Provider

Community Therapy is proud to be a registered NDIS provider, delivering high-quality allied health support to participants in The Entrance and beyond. We understand that healthcare under the NDIS can feel difficult to understand, so we’re happy to take the time with you and your coordinator to explain our services fully.

Our team works collaboratively with you, your family, and your support network to align therapy with your goals. We provide evidence-based supports across physiotherapy, occupational therapy, exercise physiology, dietetics, and home-based services — helping you to build skills, independence, participate in community life, and make the most of your NDIS plan.

Services include NDIS assessments, NDIS plan reviews, NDIS assistive technology, and more.

Dietetics – Maximising Your Wellbeing

Good nutrition plays a central role in overall health, recovery, and wellbeing. Our accredited dietitian offers tailored dietary assessment, advice, and education to support a range of health goals — from weight management, malnutrition, and chronic disease support to performance nutrition and gut health.

We take the time to understand your lifestyle, preferences, medical history, and goals so you can make sustainable changes that work for you. Whether you’re managing diabetes, cardiovascular risk, gastrointestinal concerns, or other conditions, our dietetic support is practical, empowering, and evidence-based.

Exercise Physiology

Our exercise physiologists specialise in using movement and targeted exercise to improve health outcomes, enhance physical function, and manage chronic conditions. They design personalised exercise programs that address your individual needs — helping to improve strength, cardiovascular health, mobility, and overall wellbeing.

Whether you’re managing a long-term condition, recovering from an injury, or seeking proactive support to stay healthy, our mobile exercise physiology service comes to you in The Entrance — reducing barriers to participation and ensuring therapy fits seamlessly into your life.

EP services are commonly delivered under the NDIS plan and we can even visit nearby hydrotherapy pools with you!
