Community Therapy has always offered services to older adults across our regions, through both homecare packages and short term restorative care programs before transitioning fully to Support at Home.

Who is Eligible for Support at Home Physiotherapy?

Physiotherapy under the Support at Home program is designed for older Australians who need help to remain safe, mobile, and independent at home. To access these services, you must first enter the Support at Home program through My Aged Care.

The process can be broken down into three steps:

  1. Organise an aged care assessmentStart by contacting My Aged Care, who will arrange an assessment to understand your care needs. This assessment considers factors such as your mobility, health conditions, and ability to manage daily activities.
  2. Receive a Support at Home packageAfter your assessment, you will be allocated a Support at Home package at a certain classification level. The level reflects the amount of support you are eligible to receive. Physiotherapy may be included as part of this package if it is identified as beneficial to your health and independence.
  3. Choose your providerYou then have the freedom to select an approved provider to deliver and coordinate the services in your package. We partner with all approved providers in our region to provide allied health services so if physiotherapy has been recommended, we can step in to provide personalised, in-home therapy tailored to your needs and goals.

These can increase the risk of falls, reduce confidence, and affect the ability to continue enjoying daily life. Every plan is personalised, meaning our physiotherapists consider your goals, home environment, and health conditions when designing your therapy.
